Insurance Image

We accept most vision and medical insurances. We also accept payment from flexible spending accounts (FSA) and health savings accounts (HSA), Care Credit, cash, checks, and most credit cards. We do our best to inform our patients about exam and treatment costs at the beginning of your appointment based on the information provided by your insurance companies. Exams, treatments, and products may be subject to change throughout the course of the exam depending on the diagnosis and findings by our doctors. An itemized receipt of all charges will be provided.


Vision Insurance

The staff at Illinois Eye Associates is experienced in navigating vision insurance providers. We will work with you and your vision insurance to submit claims securely and electronically. We strive to ensure that you understand your insurance coverage and will answer any questions you may have regarding your benefits. Many vision plans are designed to cover most of the basic vision-related costs. However, not all costs associated with your eye care services are insurance responsibility. Most plans often require a co-payment for an office visit and may involve additional costs that are the responsibility of  the patient. Illinois Eye Associates currently accepts Vision Service Plan (VSP), most EyeMed (EM) plans, Davis Vision (DV),  Humana Vision Care Plan (VCP), and Envolve Vision Care, in addition to most union vision benefits.  Contact our offices to find out if your insurance plan is accepted or for more information about your vision coverage.

Medical Insurance
In some cases your exam will be covered by your medical insurance.  In some situations your vision and medical insurance plans may also be coordinated to maximize your coverage and minimize your out of pocket expenses. Our staff will take the time to explain how your insurance will be billed and any co-payments you may incur.


Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA)
Illinois Eye Associates accepts payments from a flexible spending account (FSA)*.  Some employers offer this option to employees. These plans may be referred to as cafeteria plans and are elective, supplemental insurance savings plans. These plans are designed to let you save money in an account, pre-tax, to pay for additional medical expenses such as eye exams, glasses, contacts, and often laser vision surgery. Check with the benefits administrator at your employer to see if you are eligible for this program. FSAs can typically be used in conjunction with any vision insurance plan to offset any out-of-pocket costs on your behalf.

*Most FSA policies do not roll over into the next calendar year and must be used by December 31st.

A health savings account (HSA) is basically the same as a flexible spending account with the exception that funds in an HSA do not expire and CAN be carried over year after year.
